FUNCTIONSPosition SpecificCoordinates and facilitates the CFT
meetings, Intensive Case Coordination and ensures that all team
members are informed of and included in CFT meetings and follow up
communicationCoordinates the development of Child and Family Teams
(CFTs) with a focus on enhancing the involvement of natural team
members and system partnersGuides the CFT meetings through the plan
development process using accepted High Fidelity Wraparound
PrinciplesDocuments the plan development process by recording and
distributing minutes of CFT meetings, writing progress notes, and
updating Individualized Care Plans, Safety Plans, and planning
documentation under the Medi-Cal modelProvides other case
management functions as necessary, securing formal resources, and
representing the CFT progress through the wraparound process in
written documentation and verbal presentations to both internal and
external partnersWrites clear, accurate and timely progress notes
consistent with each contact/visitWorks collaboratively with all
agency programs and staff to provide support as neededAttends
weekly staff meetings, supervision and case management staff
meetings as scheduledAccountable for independent time
managementDevelops and maintains positive relationships with agency
and community programsUtilizes and maintains Microsoft Outlook
calendar with all work-related details in order to manage time
effectively and share calendar information with co-workersUtilizes
resources to utilize behavior interventions and strategiesForms

individualized wraparound plansPrepares Individualized Family Plans
and Safety Plans and coordinates each plans implementation,
encouraging the maximum use of natural resources and community
based services, and supplementing these with flexible wraparound
staffing and fundingProvides and completes timely documentation of
all delivered services, including but not limited to assessment,
treatment planning, youth and family skill building and education,
and care coordination.Provides therapeutic interventions for skill
building for example, within scope, weekly as determined by the CFT
meeting process.Clinically oversees and coordinates other services
agreed upon in the CFT meeting process acting in collaboration with
other team members and partners.Provides other care coordination
functions as necessary, securing formal and informal
